Book an appointment
Select”New Patient “ if this is your first visit. Select”Existing Patient” if you already have received any professional services from our providers. Choose the day and time you would like us to contact you for a virtual visit. Verify your demographic.

Fill out Forms
A link will be sent to your email address which directs you to our Care Portal registration page. Create your username and password and start fillint out the form

Virtual visit
Use the link provided to you and pay the allocated amount. The provider will contact you for a virtual visit.

Prescriptions
If indicated, the doctor will send your prescription to your local Pharmacy.
How to fill out Forms ?
Access the Care Portal
Once you get an email and click the form link to access our Care Portal.
With our Care Portal, you will have 24/7 secure access to educational information, prescription, labs, appointment requests, paying bill and much more.
Register
If you don't have a Care Portal account, yet, register by creating a username and password.
For added security, the password must have a minimum of 8 characters (at least 1 numeric value and 1 special character).
Fill out and Submit the Form
Fill out and submit the form prior to your scheduled televisit appointment date.
How to Join the Virtual Visit?

Confirm the Appointment
A text and email appointment reminder will be sent to you after the appointment is booked.
Confirm your appointment by clicking the confirmation link or call us to cancel or reschedule.

Make a Payment
One the day of your appointment, click the televisit link that was sent to you via text/email.
Click the pay button and enter your credit card information.
We recommend you making the payment atleast 5 minutes prior to the scheduled virtual visit.
03
Join the Virtual Visit
After payment has been made. you may join the relevisit and wait for the doctor to join.
