Reservar una cita
Seleccione "New Patient " si esta es su primera visita. Seleccione "Existing Patient" si ya ha recibido algún servicio profesional de nuestros proveedores. Elija el día y la hora en que desea que nos comuniquemos con usted para una visita virtual. Verifique su demografía.

Completar formularios
Se enviará un enlace a su dirección de correo electrónico que lo dirigirá a nuestra página de registro del Portal de atención.
Cree su nombre de usuario y contraseña y comience a completar el formulario

Visita virtual
Utilice el enlace que se le proporcionó y pague la cantidad asignada.
El proveedor se comunicará con usted para una visita virtual.

Prescripciones
Si está indicado, el médico enviará su receta a su farmacia local.
How to fill out Forms ?
Access the Care Portal
Once you get an email and click the form link to access our Care Portal.
With our Care Portal, you will have 24/7 secure access to educational information, prescription, labs, appointment requests, paying bill and much more.
Register
If you don't have a Care Portal account, yet, register by creating a username and password.
For added security, the password must have a minimum of 8 characters (at least 1 numeric value and 1 special character).
Fill out and Submit the Form
Fill out and submit the form prior to your scheduled televisit appointment date.

Confirm the Appointment
A text and email appointment reminder will be sent to you after the appointment is booked.
Confirm your appointment by clicking the confirmation link or call us to cancel or reschedule.

Make a Payment
One the day of your appointment, click the televisit link that was sent to you via text/email.
Click the pay button and enter your credit card information.
We recommend you making the payment atleast 5 minutes prior to the scheduled virtual visit.

Join the Virtual Visit
After payment has been made. you may join the relevisit and wait for the doctor to join.
